Medical Release Information - Please Read

By signing this application, I hereby give permission to The Journey Tae Kwon Do, LLC. to be medically treated or to be transported to a medical treatment facility in case of emergency. I agree to be financially responsible for my total treatment and transport

If signing for a minor, by signing this application, I hereby give permission to The Journey Tae Kwon Do, LLC. to medically treat or to transport my child to a medical treatment facility in case of emergency. I agree to be financially responsible for my child’s total treatment and transport.

Media Release Information - Please Read

The Journey Tae Kwon Do, LLC. may use my or my minor child's image including, but not limited to, photographs and/or video for use in Emails, Handouts, Newsletters, Banners, Social Media, and/or Websites. I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

To withhold this release, do not initial the Media Release portion of the application.
